Term Life Insurance

Pays the beneficiary $10,000 upon the Member's death. 

This insurance benefit is underwritten by Citizens Security Life Insurance Company.  Coverage cannot be sold to members over the age of 84.  Coverage becomes effective 90 days after the effective date of the membership.

 The principal sum reduction schedule is as follows:

  • 18-69     100%
  • 70-74     50%
  • 75-79     25%
  • 80-84     10%
  • 85+           0%

CareFlite

Fixed wing air ambulance at no charge.

Provides worldwide, 24-hour hospital bed-to-hospital bed fixed wing air ambulance service at no charge. Designed for stabilized patients who cannot receive necessary medical care where they are presently admitted for treatment. All transfers must be evaluated and approved by CareFlite in advance, and determined by a Primary Medical Authority physician to be medically necessary. Transfer must be to a specialized hospital or medical center of higher care at least 100 air miles (one way) away.

How it works
To locate the Dignity Memorial provider nearest you, call 1-800-495-1230. Provide your zip code and identify yourself as a USA+ Member. When visiting your local Dignity Memorial provider to arrange services or to preplan your wishes, just present your USA+ membership card to the funeral service professional you are working with.

Emergency Helicopter Rescue

Reimburses up to $4,000 for emergency helicopter transportation.

In the event that an eligible member suffers from a “certified injury” that requires emergency medical transportation by helicopter in accordance with EMS protocols, the program will reimburse the participant up to a maximum of $4,000.00 per occurrence.

Reimbursement includes expenses incurred from the cost of "Medically Necessary" or "Life Threatening" helicopter transportation from the scene of an accident to the nearest medical facility capable of treating the injuries or from one medical facility to another medical facility. Claims for "Medically Necessary" transports from one medical facility to another medical facility are subject to review by Lifeguard's Medical Officer.

Provisions...
One benefit will be paid per occurrence.
Benefit in excess of all other valid collectable insurance.
Coverage is worldwide.
Transportation by helicopter only.

To Use This Benefit
Call 911 or the local equivalent. Local EMS protocols will make the determination for necessity and type of medical transportation that best fits each situation.

Senior Medical Alert (Life Station)

Call any day, any time...

Using your medical alert system couldn't be simpler.  Your waterproof help button, which can be worn as a wristband, belt clip or neck pendant, is designed to be with you at all times.   In the event of an at-home emergency, simply press the help button to connect to a Care Specialist in our Monitoring Center.

Our Benefits Protector Program helps cushion the impact of economic downturns.

How you Benefit
Many individuals lose their job due to a company re-location, company downsizing or as the result of natural disasters. For most individuals, loss of employment also means a monetary loss. 

The Benefits Protector program helps cushion the impact of economic downturns that occur. Should you lose your job through no fault of your own, we will be there for you. Your membership dues will be waived and your membership benefits will continue for three (3) months. (Certain Terms and Conditions Apply. Please refer to your Membership Handbook for complete details.) 

How to Use Your Benefit
1. Notify USA+ within 20 days of the loss of employment. Notice must be submitted to
     USA+ at the following address:

 2. Upon receipt of your written request, USA+ will send you a Benefits Requests
     application to complete and return to USA+. 

3. When the completed form is received, USA+ will contact your former employer and
     validate the reason for separation. 

4. If the employer confirms that the loss of employment was not the result of misconduct
     connected with your work and you did not voluntarily quit, you will receive a letter from
     USA+ confirming the continuation of your USA+ membership. 

5. If the employer is not able to confirm your loss of employment, you will receive a letter
     from USA+ stating that we are unable to continue your USA+ membership.

Rewards Program - SL10

The privileges of being a USA+ member only get better and better. In addition to access to great healthcare benefits, USA+ members are automatically enrolled in our Member Reward Program.

One you have been a Silver Life 10 member in good standing for at least ten years, you can receive a 100% refund of all membership dues paid to USA+.  What that means to you is that you will receive a reward of the amount listed below, based on the plan you purchased.

10 year reward - $10,080

No action is required to activate your reward program. To redeem your reward, contact USA+ at 800-USA-1187.

Member is only eligible for one reward per qualifying term. Once the reward has been redeemed, the eligibility period begins again.
