Life Protector

Pays the beneficiary $10,000 upon the Member's death.

Reduction in the Principal Sum for ages 70 and up. This insurance benefit is underwritten by Citizens Security Life Insurance Company.

You can receive financial protection during the emotional time of a diagnosis of a life threatening illness, such as cancer, heart attack, kidney renal failure, major organ transplant or stroke.  Critical Illness insurance helps fill the gaps not covered by traditional health disability, life and accident insurance.  If a life-threatening illness occurs, our Critial Illness benefit pays the insured a benefit to help protect his or her quality of life.

Coverage is not provided for loss due to a pre-existing condition for 12 months from the Covered Person's membership effective date.  Coverage is not provided for members over the age of 69.  This benefit is provided to USA+ members by the group policy issued by an A.M. Best rated insurance company.  Certain terms and conditions apply and benefits are subject to the Exclusions and Limitations.  See your Membership Handbook or Certificate of Coverage for the details.

This benefit entitles you to:

• No initial consultation fee.
• One 30-minute in-office visit per month.
• Preparation of a simple Will or review of an existing Last Will and Testament at no charge.  (one per membership)

How it works
To locate the Dignity Memorial provider nearest you, call 1-800-495-1230. Provide your zip code and identify yourself as a USA+ Member. When visiting your local Dignity Memorial provider to arrange services or to preplan your wishes, just present your USA+ membership card to the funeral service professional you are working with.

How you Benefit
Many individuals lose their job due to a company re-location, company downsizing or as the result of natural disasters. For most individuals, loss of employment also means a monetary loss. 

The Benefits Protector program helps cushion the impact of economic downturns that occur. Should you lose your job through no fault of your own, we will be there for you. Your membership dues will be waived and your membership benefits will continue for three (3) months. (Certain Terms and Conditions Apply. Please refer to your Membership Handbook for complete details.) 

How to Use Your Benefit
1. Notify USA+ within 20 days of the loss of employment. Notice must be submitted to
     USA+ at the following address:

 2. Upon receipt of your written request, USA+ will send you a Benefits Requests
     application to complete and return to USA+. 

3. When the completed form is received, USA+ will contact your former employer and
     validate the reason for separation. 

4. If the employer confirms that the loss of employment was not the result of misconduct
     connected with your work and you did not voluntarily quit, you will receive a letter from
     USA+ confirming the continuation of your USA+ membership. 

5. If the employer is not able to confirm your loss of employment, you will receive a letter
     from USA+ stating that we are unable to continue your USA+ membership.

One you have been a Life Protector member in good standing for at least five years, you can receive a 50% refund of all membership dues paid to USA+, or you can wait to redeem 100% after 10 years.  What means to you is that you will receive a reward of the amount listed below, based on the plan you purchased and the length of your membership in USA+.

No action is required to activate your reward program. To redeem your reward, contact USA+ at 800-USA-1187.

Member is only eligible for one reward per qualifying term. Once the reward has been redeemed, the eligibility period begins again.
